Complete these 5 steps

(1) Register for CHRVA HERE (both practice team and elite team members must register; practice team should select ECE as their practice team)

(2) Complete the 2016-2017 USAV YOUTH & JUNIOR VOLLEYBALL PLAYER MEDICAL RELEASE FORM, available HERE.

(3) Click HERE to pre-pay via PayPal  ($40 tryout fee, $50 at the door).

(4) Fill out our registration form below.

(5) Bring to registration:

• CHRVA Registration Card

• USAV Medical Release Form

• ***If you do not prepay by using the PayPal link, bring $50 payable by cash or check

14s, 15s and 16s: November 11, 7pm-9pm
17s: November 13, 3pm-6pm (make up date for 14s, 15s, and 16s)

Supplemental Tryouts: November 15, 7pm – 9pm
