- STEP 3: Pay Tryout Fee ……………………………..–>> Pay HERE (Non-refundable tryout fee is $40 per player online via PayPal by Nov 6, 2019)
- STEP 4: Fill in USAV Medical Release Form –>> Complete HERE
- STEP 5: Bring a copy of STEP 1 & STEP 4 to tryout (plus $50 cash if your tryout fee is not paid via PayPal in full by Nov 6, 2019)
